Perform the following steps:

  1. Download the UtilityPRO RMA spreadsheet (Rev 2.4 updated on 9/28/2011), which is in Microsoft Excel format.
  2. Open the spreadsheet and enter the UtilityPRO RMA parts, serial numbers, failures, and notes.
  3. Save the spreadsheet and attach it to this RMA form using the field below.

download the UtilityPRO RMA Instructions (Rev 2.0 updated on 12/10/2010).
Attach the UtilityPRO RMA Spreadsheet:
 To attach the RMA spreadsheet, click the Browse button, navigate to the directory where the file is located, select the file, and click the Open button.
The submitted file must be less than 2 MB. To submit multiple files, combine them in a ZIP file.

IMPORTANT

There will be a $90.00 evaluation fee charged for any warranty or non-warranty items deemed Non-defective or Non-repairable.

*** Please use proper packaging when shipping the UtilityPRO thermostats back for repair to ensure that no damage is done to the product.
If the product is damaged during shipping because of improper packaging, the customer is responsible for the replacement costs.

If the product is NOT UNDER WARRANTY, by submitting this form, you agree to the following terms:

  • If the product is Not repairable and you do not want it replaced, THERE WILL BE A $90.00 EVALUATION FEE CHARGED AND THE PRODUCT WILL NOT BE RETURNED TO YOU.
  • If the product is repairable, there will be a repair price charged.
  • If the product is Not repairable and you want it replaced, please provide a PO# .
  • If you would like to be notified, please provide the name and number of someone to contact in the special instructions text box.
